Scheduling an Appointment

The Eye Clinic for Animals sees patients by appointment only. We are currently accepting appointments for small animals only (dogs, cats, pocket pets, exotics). We are currently not accepting large animal appointments (horses and livestock). 

 

New Patients

The Eye Clinic is booking out new patients weeks to months. As a result, submission of the form below and medical records are required to schedule an appointment. Once received, we carefully review all information in an attempt to offer you and your pet the most appropriate and timely scheduling option.

 

  • Start the process by filling out the form below.

  • Next, contact your referring veterinarian (general practitioner or other specialist) and have them send us medical records for all eye issues, as well as general medical records for the past 1 year.

  • We will then contact you with appointment options.

 

Returning Patients

Returning patients are encouraged to schedule follow-up appointments while in-office after a visit. To facilitate this, our appointment schedule is typically available 6-12 months in advance. If you are unable to schedule while in-office, we are happy to receive your request at a later date. Please call the clinic at (360) 866-0931 -- There is no need to fill out the form belowPlease ensure that the Eye Clinic receives all updated pertinent records from your referring veterinarian in a timely fashion.​​

Client Conduct and Cancellation Policy*

The Eye Clinic for Animals strives to provide a welcoming, positive, and safe environment for all creatures who pass through our doors. As a result, the following will not be tolerated, regardless if directed at a human or animal:

  • Verbal abuse: malicious or harmful statements about others, using profanity

  • Any form of harassment

  • Making threats or using intimidating tactics

  • Allowing your pet to intimidate or harass another person or patient

 

For the safety and well-being of our staff, clients, and patients, the above will be strictly enforced. Failure to comply may result in your being asked to leave the property, involvement of law enforcement, immediate cessation of clinical services, and/or irrevocable dismissal from the practice.

Should you need to cancel a medical or surgical appointment, we request 2 business days' notice. This means that appointments scheduled for a Monday or Tuesday must be cancelled by the end of the previous work week on a Thursday or Friday, respectively.  The office is closed by 1pm Friday afternoons. Voicemails or other notices left over the weekend will be treated as insufficient notice. If 2 business days' notice is not received, a cancellation fee of $75.00 will be applied for missed exam appointments, $225.00 fee for missed surgery appointments.  Cancellation fees must be paid in full prior to rescheduling.
